Dr Jonathan Farr DSc, Chief Clinical Officer of Advanced Oncotherapy plc, presents the results of a recent study undertaken with partners the Cleveland Clinic, which demonstrates the potential advantages of treating breast cancer with the LIGHT system, when compared to conventional cyclotrons. The study demonstrates that LIGHT proton minibeams could result in up to 99% reduction in radiation dose to critical organs, the heart and lungs, compared with cyclotron-generated protons.

On 26th September 2022 Advanced Oncotherapy reached a critical milestone in our journey in bringing a new linear accelerator proton treatment system to the market - the acceleration of the proton beam up to maximum system energy of 230 MeV.

Advanced Oncotherapy plc reported new findings demonstrating the potential ability and superiority of its next-generation proton therapy system, LIGHT, to treat resistant cancers. The findings build on research at the Mayo Clinic that highlighted the benefits of combining proton therapy (PT) with targeted small molecule damage repair inhibitors.

This study aims to investigate the use of proton Pencil Beam Scanning (PBS) for uniform FLASH irradiations in clinical and (radio)biology settings using a pulsed proton therapy Linac and to report on PBS FLASH beam delivery possibilities and limitations.

The purpose of the study is to determine which clinical indications benefit most and by how much from minimizing and holding a proton scanning spot size constant over the range 150-230 MeV in comparison to existing systems and techniques.

Treatment planning for proton therapy is complex and requires a number of steps to achieve the desired dose distributions. Without a national or international strategy to coordinate unified treatment protocols, patients treated with protons have been managed with several delivery modalities, treatment paradigms and fractionation schemes. A key area for optimising the treatment planning process is therefore in gaining experience by encouraging all centres to share treatment plans with each other, retrospectively and in real time. To facilitate this process, Advanced Oncotherapy is working on treatment planning software to be hosted on the cloud.

The combination of proton therapy with immunotherapy is considered promising and has gained substantial interest over the last 2 years. According to Prof Justin Stebbing, an internationally acclaimed expert in immunotherapies for cancer, “the synergies between immunotherapy and radiotherapy can potentially allow for better treatment of cancers, improving both quality and quantity of life.” A recent paper published in the British Journal of Radiology shows that proton therapy might be better adapted to for combination with immunotherapy because it causes less damage to the immune cells and induces a stronger immune response due to a different cell death mechanism.

FLASH continues to prove its unique advantages in healthy tissue sparing with tumour control even in radioresistant cases. The LIGHT system is uniquely suited to application of FLASH at all depths.
